Orthognathic surgery includes not only jaw surgery but also all other facial surgery that can restore abnormal face to normal face. In order to get perfect result, clear goal must be set at first and the most appropriate surgical armamentarium should be selected. Goal of the most esthetic face might be different in different places at different times. It means it strongly require individualization. How it can be done?
First, enough discussion about the goal of the esthetic face among surgeon, orthodontist, and patient must be done. And this discussion is supposed to be done in common terms. A new modified classification of the abnormal face will be presented.
Second, the face should have clear 3 concavities on the profile and appropriate vertical proportion of upper, middle and lower face, without any asymmetry. These are the basic surgical goal.
Third, the most efficient surgical options must be selected. Then, it requires experience to do surgery without any complications and to get the most esthetic face.
