Sagittal split ramus osteotomy is the workhouse to correct mandibular deformities and could be the most popular orthognathic procedure. A successful split relies on a natural split pattern of a normal anatomy of a mandible. Complications may occur especially when the anatomy deviates from normal. Bad split, nerve injury, haemorrhage and poor positioning of the bone segment affect the clinical outcome as well as causing morbidities. It is important first to understand these complications, and then the management when they occur. Preventing these complications is the key of success for a satisfactory outcome of orthognathic surgery. This lecture aims to share with the audience of some tips and tricks on the prevention and management of sagittal split ramus osteotomy.
